MEMBERSHIP TERMS & CONDITIONS: This membership and services purchased under this membership are non-transferable to any other person or entity. Memberships are non-refundable. For purposes of identification and billing, you agree to provide us with current, accurate, complete and updated information including your name, address, telephone number and applicable payment data. You agree to notify us promptly of any changes. We reserve the right to revoke membership at any time for misconduct on our premises. We reserve the right to change pricing at any time upon reasonable notice. Should prices increase, you have the right to cancel membership.
Appointment Cancellations done within 24 hours of your appointment are subject to a cancellation fee of up to 100% of the missed appointment cost. All no-shows are charged 100% of the appointment fee. Cancellations within 24 hours and no-shows are subject to removal of an accrued session.
Memberships can be cancelled at any time. All membership cancelations require 30 days written notice. Payments due prior to the effective date will be charged as scheduled. We reserve the right to terminate or deny re-enrollment for an indeterminate amount of time if you have an unsatisfactory payment history. If through no fault of ours, your payment account does not contain sufficient funds to complete the transaction, or your payment account or credit card does not otherwise permit the transaction to be executed, you will be charged a $35 insufficient funds fee. We will contact you to update your account with a working payment method. Upon termination of your month to month membership, accumulated member service packages expire and are non-refundable 90 days after you cancel your membership.
You must be a member for at least 30 days before you may freeze your membership. A minimum of one EFT payment must have been made before a membership is eligible to be frozen. Memberships can only be frozen two times in a 12 month period for a maximum of 3 months at a time. A membership must be unfrozen a minimum of 30 days before it can be frozen again. Members can redeem accumulated services during a freeze but cannot utilize any additional membership benefits.
EXAMPLE: Sign-up/First Payment on 10/11/23. First Automatic Payment charged on 11/11/23. A 60 Day freeze is requested on 5/13/23 and processed on 5/23/23. Automatic payments scheduled between 5/23/23 and 7/21/23 will not be charged, this would include regular installments scheduled to take place on 6/11/23& 7/11/23. Membership benefits would also be suspended from 5/23/23 until 7/21/23, and would resume on 7/22/23. The next automatic payment would then be charged on 8/11/23.
